2024-2025 Religious School Registration

This form is for non-member enrollment in our religious school program. If you are a member, please log into your account and enroll from your member dashboard. If you have questions, please contact the office at (916)988-4100. 
Please include information for each child registering for our program. 
You will need to choose the tuition category for each child. If you have a 7th grade student who will celebrate bar/bat mitzvah this year, you need to choose both the 4th-7th grade category AND the B'nai Mitzvah category. If you will need a scholarship this year please indicate this in the field provided.

In case of an accident or sudden illness, when a parent or guardian is unavailable, I authorize a Temple Or Rishon representative to obtain medical care for my child, including necessary transportation, in accordance with their best judgment.  I further authorize the doctor named above to provide the care and treatment he/she considers necessary.  If the physician designated above is unavailable, I authorize such care and treatment to be performed by a licensed physician selected by the temple representative.  I agree to pay all costs incurred as a result of the foregoing.
